Civil status certificate (eIDAS)

Use this form to request a transcript (deed) from the civil registry. This is not the same as an extract from the Basic Registration of Persons (BRP).

An extract (deed or copy) from the Civil Registry

• Contains data on any of the following events: birth, death, marriage, partnership, divorce.
• Please apply to the municipality where the event took place.

Examples:

1. You live in Zoetermeer, but your child was born in Leiden. You must then request the birth certificate from the municipality of Leiden.  
2. You live in Zoetermeer and are divorced. You are married in The Hague. You must then request the divorce certificate from the municipality of The Hague.

An extract from BRP (Basic Registration of Persons).

• Contains personal data such as name, address, place of residence.
• Apply in the municipality where you live.
• Also called "extract of population register.
• Is requested, for example, by a housing association.
