Important information before proceeding

Attachments to be included

This application requires you to include a number of attachments. Make sure you have these attachments on hand digitally before completing the form. Keep in mind:

  • You must submit the documents for this application in the following file formats: Word, Excel or Pdf.
  • A photo or screenshot of a document is not allowed.

What to include:

  • A map (with scale reference) of:
    • the property (including terrace)
    • the location and layout
    • the purpose of the spaces to be distinguished
    • the boundaries of the premises of the catering establishment
  • A copy of a valid ID of the business owner(s)/director(s) and all salaried executives. Are you making this application on behalf of someone else? Then an authorization and a copy of a valid ID of this person(s) is also required.
  • A copy of the employment contract of all salaried executives.
  • Optional: a declaration of no interference. This allows managers to declare that they have no involvement in the management or operation of the catering or liquor company. These persons are not required to have a Declaration of Social Hygiene.
  • Statements of Social Hygiene do not need to be sent along. We look these up in the register ourselves. However, managers must have this certificate.
  • For associations or foundations: a copy of the board rules.
