You are applying for the SEBO seal of approval on behalf of your organization (hereinafter referred to as "client"). You declare that you are authorized to act on behalf of the organization.

By submitting the form, you agree to:

• The content of the Hallmark criteria.
• Your application will be submitted to the SEBO seal of approval holders.
• Audit by the Keurmerkinstituut B.V. in Zoetermeer.
• The regulations and general terms and conditions of the Hallmark Institute.

You must send attachments with this form. Make sure you have these attachments (if applicable) ready digitally before filling out the form:

1. A description of your organization.
2. Extract from the Chamber of Commerce (not older than 1 year).
3. Recent annual report (if available).
4. Multi-year policy plan.
5. Details of contact person at the Werkgeversservicepunt (WSP) Zoetermeer.
6. An overview of external cooperation partners.
7. A detailed description of the measures (to be) taken to encourage the entry and advancement of vulnerable groups within the company.
8. An overview of the workforce broken down by age, gender, ethnicity and work disability.
9. An overview of number of unpaid efforts on behalf of Zoetermeer social and community causes.
