A recognition cannot be undone.
At the time of recognition, the recognizer must bring a valid ID.

cannot
use this form if the gender name of the acknowledger is chosen when acknowledging the
first
child. Both the mother and the acknowledger must be present in person when the child's name is chosen.
The birth mother must not be married or have a registered partner with someone else.
The recognizer automatically gets custody. Do you not want this? Then make an appointment to declare together that only mother will have custody.
With this form, you only authorize the recognition. The recognizer must make another appointment to sign the deed.
