What documents should you include?

You must enclose attachments with this operating license application. It is wise to have these documents at hand (digitally) before completing the form. The process is faster if you submit the application and attachments as complete as possible. The documents for this application can only be submitted in the following file formats: Word, Excel or PDF. This means, that a photo of a document is not sufficient.

With the application, please send along:

• A business plan that states:
  • the nature of the business
  • the form of exploitation
  • the financing of the business
  • the target audience
  • the products offered
    
    
• A lease, purchase or rental agreement of the property.
  
  
• A map of:
  • the property (including terrace)
  • the location and layout
  • the purpose of the spaces to be distinguished
  • the boundaries of the premises of the catering establishment
    
    
• A copy of a valid ID of all executives who are employees.
  • Are you making the application on behalf of someone else? If so, a proof of authorization and a copy of a valid ID of this person/these persons are also required.
    
    
• A copy of the employment contract of all salaried executives.
  
  
• A certificate of good conduct of the applicant(s) and supervisors.
  
  
• A completed Bibob form, including attachments (more information: Bibob). You send this form separately and do not need to upload it in this application.
