Important information before proceeding

Attachments to be included

You must send attachments with this application. It is wise to have these documents on hand (digitally) before you fill out the form. The process can go more smoothly if you submit the application as completely as possible. Keep in mind:

  • You must submit the documents for this application in the following file formats: Word, Excel or Pdf.
  • A photo or screenshot of a document is not allowed. 

You must include the following documents:

  • A floor plan of the premises showing the location, layout, purpose of the rooms to be distinguished and the boundaries of the premises of the catering establishment (including terrace).
  • A copy of the identity card or passport of the business owner/directors and executives. If you are making this application on behalf of someone else, the authorization certificate and a copy of passport of that person(s) is also required.
  • A copy of executives' employment contracts.
  • Optional: a declaration of no interference. This allows managers to declare that they have no involvement in the management or operation of the catering or liquor company. These persons are not required to have a Declaration of Social Hygiene.
  • Statements of Social Hygiene do not have to be sent along. The municipality can look up these declarations in a register. However, managers must have this declaration.
  • For associations or foundations: a copy of the board regulations.
  • A completed Bibob form including its attachments. You can fill out the form digitally via this link. You do not need to upload the form and attachments in this form.
