Report missing passport or identity card

Use this form to report your stolen or lost passport or ID card.

• Your passport or ID card will be blocked immediately. The document is then invalid and can no longer be used. 
• You will need DigiD to complete this form.
• You can only report your own passport or ID card as stolen or missing.
• After reporting it, you can make an appointment online to apply for and pick up your new passport or ID card.

Do you find your passport or ID card? Then you must return it to us as soon as possible.

Missing three or more times?

Has your passport or ID card gone missing 3 or more times in 5 years? Then your data may be recorded in the Register of Passport Signals (RPS). The application for your new passport or ID card will then be stopped for a while. This may take 4 to 8 weeks. The outcome of the investigation may be that you will not receive a passport or ID card. Also, if your passport or ID card is found after you reported it missing, this will count.
