Identity card
You must always apply for and collect an identity card (ID card) in person. This also applies to children. From the age of 12 a child may apply for and collect his/her own ID card.
Children under 12 can apply for an ID card themselves, but only with the permission of all persons with parental authority. Please use the form below:

What do you need to apply for an ID card
- You are registered in the municipality of Zoetermeer.
- All identity cards and passports you still have. Even if they have expired.
- Your foreign travel document (if you have one).
- A good-looking color passport photo.
- At the time of application, the photo must be no more than 6 months old.
- Passport photo requirements are attached to the photo .
- A headscarf is allowed, but the face and eyes must be fully visible. Also, the headscarf must be even and single-colored. It also has to be in contrast with the background.
- Children under 12 can apply for an ID card themselves but only with the permission of all persons with parental authority.

- All persons with (parental) authority complete the online form below.
- Consent given online is valid for 3 months.

Giving consent in person
- All persons with (parental) authority come to the appointment with the child and give permission at the front desk.
- They bring a valid ID or passport.
Giving written consent
- Download the form"Consent passport or ID card for minor child" (pdf).
- All persons with (parental) authority complete and sign the form. You may also prepare your own statement.
- The child brings the completed and signed form + identification documents of all persons who completed the form to the appointment.
If one of the persons with (parental) authority is unable or unwilling to give consent, you can request substitute consent from the court in The Hague.
What do you need to pick up an ID card
- The pickup receipt you received during the application.
- All travel documents that need to be turned in.
- A valid identification document.
- Children up to 12 years old must come together with one of the persons with (parental) authority to collect the identity card.
- Children 12 years and older may come to pick up their identity card themselves.
Rush ID card application
Need a new ID card quickly? Then use the rush application. Please keep in mind the additional fees.
- Call 14 079 to make an appointment.
- There is no need to make an appointment for pickup.
- After your appointment at the front desk, the rush application goes into effect.
- Application before 2 p.m. at the counter > pick up ID card the next business day from 11 a.m. onwards.
- Application after 2 p.m. at the counter > pick up ID card after 2 working days.
ID card lost, stolen, damaged or unreadable?
ID card lost or stolen? Report it as soon as possible.
Reporting online
- Your ID card will be blocked immediately.
- File a report using your DigiD.
- You can only report your own ID card as missing.
- After the declaration, you can apply for a new ID card.

- Your ID card will not be blocked until the time of the appointment.
- You must make an appointment for this.
- At the front desk, a statement of missing is made.
- After the declaration, you can apply for a new ID card.
Appointment to report missing ID card
Has your passport or ID card gone missing 3 or more times in 5 years? Then your data may be recorded in the Register of Passport Signals (RPS). The application for your new passport or ID card will then be stopped for a while. This may take 4 to 8 weeks. The outcome of the investigation may be that you will not receive a passport or ID card. Also, if your passport or ID card is found after you reported it missing, it still counts.

- Step 1 > Click on the link below to schedule your appointment.
- Step 2 > Then on the right side of the screen under 'Apply for/Pick up passport and/or ID card', indicate how many passports and/or ID cards you want to apply for during the appointment. Would you like to request only a passport? Then choose number '1'. Would you also like to apply for an ID card during the same appointment? Then choose number '2' (1x an ID card and 1x a passport).
Example 1: You are coming together with your partner, you both want to apply for an ID card, then choose number '2'. If one of you both also wants to apply for a passport, then you choose number '3' (2x an ID card and 1x a passport).
Example 2: You are a family of 2 partners and 2 children. You want to request an ID card for all members of the family. You then choose number '4' (4x an ID card). If you also want to apply for a passport for everyone, then choose number '8' (4x an ID card and 4x a passport).
- Step 3 > Would you also like to apply or renew a driver's license or request a transcript of civil status during the appointment, for example? You can! Click on 'Add service' above the agenda and choose what else you want to arrange or request during your appointment.

Notice! You must pick up a requested ID card within 3 months or it will be destroyed.

Change or delete appointment?
Once you have made an appointment you will receive a confirmation e-mail. This e-mail also contains a link that allows you to change or delete your appointment.
Do you no longer have the e-mail? Call 14 079. A staff member can change or delete the appointment for you.

You pay the fee when you apply for your ID card. At the counter, you can only pay by debit card or cash.

Applying for ID card after naturalization
You can do this 5 days after you have been at the naturalization ceremony.

Validity
- An ID card is valid for 10 years.
- An ID card for children up to 18 years old is valid for 5 years.

Are you traveling to England, Scotland, Wales or Northern Ireland as of October 2021?
As a result of the Brexit, as of October 1, 2021, you can only travel to the United Kingdom with a passport. An ID card will then no longer be valid. The countries covered by the United Kingdom are: England, Scotland, Wales and Northern Ireland.

Logging into the government with your ID card
ID cards issued from January 4, 2021 will have an additional feature that allows you to log in to the government, education, healthcare or your pension fund. After collecting your new ID card from the municipality, you will receive a letter from DigiD within 5 working days. With this you can activate the login function.
