Birth declaration

You must register your baby within 3 days of its birth (the day of birth does not count). You must register in the municipality where your child was born.

Was your child born in Zoetermeer? You can register the birth online. This can only be done by the legal parent and when one of the parents has the Dutch nationality. Is this not the case? Call 14 079 for an appointment.

Prefer to come to City Hall? Then make an appointment online for the declaration of birth.

Who can file reports?

The following persons may report the birth of a child:

  • the legal father or mother either
  • someone who was present at the birth (only if the mother or father cannot file a declaration) either
  • someone from the institution or residence where the child was born (only if no one was present at the birth).
Choice of last name

As parents, you decide which last name your child will have. You can only choose the last name for your first child. You can do this before the birth or at the time of the birth declaration. All subsequent children in the family will automatically receive the same last name as your first child.

Other nationality

Does your child not have the Dutch nationality? Then you may not be able to decide which surname your child will get. This depends on the country of your child's nationality. Ask at the embassy or consulate.

Birth certificate

The municipality makes a record of every birth that is declared. The birth certificate includes:

  • the first name or first names and surname of the child
  • the date, time and place of birth
  • the names of the parents

Residence permit for your child

Was your child born in the Netherlands and does not yet have a residence permit? Then check the Immigration and Naturalization Service (IND) website for more information about the conditions and the application procedure for a residence permit.

Declaration of birth of deceased child or child born lifeless

Deceased child

Did your child live for a short time after delivery, but died before birth registration? If so, a birth certificate is first drawn up in the municipality where your child was born. Then a death certificate is prepared in the municipality where your child died.

Report

Online birth registration can only be done by a legal parent and when one of the parents has the Dutch nationality. In this form we will also ask you immediately about recognition of the child.

Making an appointment
  • You must file a declaration within 3 working days of the birth (day).
  • You cannot file a tax return on Saturday, Sunday or a national holiday. In that case, you will be given an additional 1 business day to file a tax return.

Change or delete appointment?

Once you have made an appointment you will receive a confirmation e-mail. This e-mail also contains a link that allows you to change or delete your appointment.

Do you no longer have the e-mail? Call 14 079. A staff member can change or delete the appointment for you.

Bring
  • valid proof of identity of the person making the declaration
  • valid ID of the mother
  • statement from the doctor or midwife (if you have one)
  • marriage certificate (not required)
  • certificate of recognition of unborn child (if you have one)
  • deed of choice of name (if you have one)

Child born in lesbian relationship

The mother and duo mother who are married or in a registered partnership bring a parental statement if the donor is anonymous or the acknowledgement certificate unborn child.
