Register for Multidisciplinary Team (MDT)

Here you can register a resident or family for the Multidisciplinary Team (MDT) of the municipality of Zoetermeer. The MDT supports residents and families with multiple problems by drawing up a single integrated plan of action, managing the assistance involved, and coordinating cooperation between different departments and chain partners. The case manager provides a permanent point of contact for the resident.

When to register?

The MDT offers an integrated approach when regular coordination between the parties involved does not lead to a breakthrough. Multiple problems exist when:

  • At least two legal frameworks apply (e.g., the Social Support Act and the Youth Act, or Debt Assistance and the Participation Act).
  • Multiple departments within the Social Domain and/or Security are involved.
  • Coordination and direction are needed to arrive at a coherent plan of action.

The MDT meets weekly and consists of experts from various departments within the Social Domain and Security.

Who can register?

Residents cannot register themselves with the MDT. Registration must be done by a professional involved, such as:

  • Neighborhood team worker
  • GP
  • Social worker
  • Debt service provider
  • Other involved care providers or chain partners

What is expected of the applicant?

  • You assess whether there are multiple issues within multiple legal frameworks.
  • You discuss the possibility of registration with the resident.
  • You explicitly request permission to share relevant information with the departments concerned.
  • You submit the registration form.

Because information is shared between different departments within the MDT, consent from the resident (and, if applicable, family members) is required.

How does MDT work?

When a decision is made to address your problems in the MDT, staff from one or more departments will meet with the resident and/or their family. Together, they will draw up an action plan. The resident will be assigned a case manager.

The MDT discusses cases on a weekly basis and feeds back the discussion to the resident via the case manager.
