Informal Care Declaration - Moving for Informal Care

The informal care declaration must be requested by the person who already lives in Zoetermeer. If both of you live in Zoetermeer, it doesn't matter who submits the request.

Help with filling out the form

Please contact the Zoetermeer Forms Brigade. Every morning from 9:00 a.m. to 12:00 p.m., there is a walk-in consultation hour; you can stop by without an appointment. The address is: Croesinckplein 24 (Entrance B) in Zoetermeer. Would you prefer to come in the afternoon? Then please make an appointment by calling 079 – 320 03 77.

You will receive an informal caregiver certificate if you meet the following requirements:

• A person who needs care cannot function independently or participate in society without assistance. This may be due to old age, an illness, or a disability, for example.
• You know each other personally. For example, you are partners. Or you are family members, friends, or acquaintances.
• The caregiver is not paid.
• At least 8 hours of care per day are needed, and this need will not decrease.
• The informal care has been going on for at least 3 months, or will continue for at least another 3 months.
• If you lived closer to each other, that would make providing informal care easier.

An informal caregiver declaration is free.

• We will process your request within 6 weeks.
• You will receive the decision by post.
