Moving from another municipality to Zoetermeer
Are you moving to Zoetermeer soon? And do you have help or an aid through the Wmo where you currently live? If so, you may be able to continue receiving this help (facility). Keep in mind that we will do additional research.
These include:
- Household assistance.
- Assistance with transportation, such as a wheelchair, mobility scooter or the Regiotaxi.
Moving within Zoetermeer
Do you already live in Zoetermeer? And are you moving to a different residence within Zoetermeer? If so, please contact the Wmo department to discuss the possibilities.
Moving from Zoetermeer to another municipality
Are you moving to another municipality? If so, you must notify the Wmo department in time.
- Contact the Wmo Department.
- You will discuss with our staff member whether you may take or return the device.
- Did you buy a device with a personal budget (PGB) or allowance? If so, you may have to repay part of the amount.
How it works
We use the research previously done by the municipality where you currently live. We also look at how you will live in Zoetermeer.
Let us know 8 weeks before the date of your move that you have Wmo assistance or an aid from the municipality where you currently live.
Can I bring tools to Zoetermeer?
When you move, you may bring aids with you. We will then make arrangements with the supplier of your aid(s). However, you must make your own arrangements for proper storage of an aid such as a mobility scooter or tricycle.
Do you have household help?
As a municipality, we use the "Powerful ageing" approach. Residents who want help with housework first participate in exercises to enable them to do the housework themselves for as long as possible.
Are you receiving household help in the municipality where you currently live? Then we will see if you can join these exercises after you move. More information can be found on the HRC Powerful Ageing website.
When can you not use this scheme?
If the help or aid expires within 3 months of the move date. We will then review your situation first. You can file a Wmo notification.
Help via Personal Budget
Are you receiving the help or aid via a Personal Budget (PGB)? You can file a Wmo notification.
Home modification or other WMO provision
Do you have an adaptation to your home? Or another Wmo facility not mentioned here? You can file a Wmo notification. Look for more information at:

Individual counseling or day care
Do you receive individual assistance or day care? Then you do not need to apply for this at the municipality. For this you need to contact inZet yourself. Look for more information at:

Let us know if you have Wmo assistance or an aid from the municipality where you currently live. Simply use the form to do so:

Complete the form 8 weeks before the date of your move.

Enclose with the form
With the form, you will need to include a number of attachments. Make sure you have these ready digitally before filling out the form:
- The letter from the municipality where you currently live stating what help you will receive and until when (the "Wmo indication decision").
- The report of the investigation (the "kitchen table interview") from the municipality where you currently live.
Do you no longer have the letter or report? Ask your contact person at the municipality where you now live to send the letter and/or report to you.

- The details of your contact person at the municipality where you currently live (name, phone number and email address).
